Personal Identifying Information
Applying for a medical card can be a daunting process with a large volume of information required in order to complete an application. Applicants must provide their social security number and date of birth. In order to receive the medical card, an individual must be either a US citizen or a legal alien. A birth certificate or a copy of a naturalization certificate will be required as part of a medical card application process for citizens. For legal aliens, some form of documentation will need to be provided in order to prove that they are legally eligible to be residing in the United States. In order to apply for a medical card, individuals must prove they are a resident of the state in which they are applying for medical assistance. Generally, a driver's license or state identification card will be accepted.
Income and Assets
Applicants for a medical card must prove their eligibility for medical assistance. Individuals or families must prove that they meet the income and assets eligibility requirement to their particular state. Documentation of any income must be provided. Types of income include: employment, self-employment, disability, SSI, retirement income, alimony and child support. Pay stubs, copies of checks and/or tax returns might be required in order to verify income for a medical card application.
Household Details
Household information must also be provided on a medical card application. Applicants must list details about everyone living in a household including children who are under 18 years of age. Medical card applications will ask applicants to provide details on any member of a household who is not currently living in the house. Details about an absent parent will need to be provided on the application as well.
Additional Household Details
In some cases, individuals or families who do not meet some of the income requirements can still qualify for the medical card. All applicants will be asked whether anyone in the household has a disability or severe medical problem. The application will require a woman to answer whether she is pregnant. The application will also ask if anyone in the household is attending school. Other factors affecting eligibility include: expenses paid out for care of an adult who needs care while the head of household is working or attending school.
