1. Log onto cookcountyvitalrecord.com.
2. Choose Birth Certificate, select the city you were born in, then click Continue.
3. Select My Birth Certificate. Fill out your first, middle and last name as it appears on your birth certificate, then click Continue.
4. Enter your date of birth, then click Continue.
5. Select the reason you'll be using your birth certificate, then click Continue.
6. Choose which type of certificate you want--regular or expedited,--then click Continue.
7. Fill out the final few steps, including parents' names and your gender, to confirm that your identity. Proceed to checkout and select the type of shipping you want.
8. Fill in your credit card information and billing address, and your birth certificate will be shipped to you.
Ordering in Person
9. Visit a Cook County Clerk's office nearest you. Find the offices by checking cookcounty.com.
10. Request a copy of your birth certificate when you speak with an employee.
11. Bring a copy of your driver's license or other picture ID and your social security card, as well as personal information that's found on your birth certificate, such as father's home city, mother's maiden name, and your city of birth.
